Crypt of the Necrodancer is a game developed by Brace Yourself Games, released on 25.04.2015. It's available for purchase on Steam and GOG.

The sigil was added in the update on 29.05.2015.

Sigil

The sigil is located as an eye.png file in the root folder of the game.

It does not appear in the game itself in any way.

Piece

The piece is embedded in the sigil file itself as a GIF image.

No steganography is used. The image data is just in the end, after the tail IEND chunk of the PNG.
